Note: If you do not have your Laboratory username and password, please contact Reda Ozuna, Accreditation administrative business partner at [email protected]. Please prepare each file in an Adobe portfolio or PDF file with attachments embed within the PDF file. Each file should be no larger than 10 MB. If you feel the laboratory response will be larger than 3 or more 10 MB files, please submit by Dropbox to  [email protected] a Dropbox link.

The audit team members and Accreditation Committee will receive access to the documents for review. (The preferred format to use for your Laboratory response is an Adobe PDF portfolio file. For example, and instructions click here).

For best results please do not use Explorer browser. All others seem to work successfully.

1. Go to www.aavld.org and log in, with your Laboratory username and password.

2. Select "My Profile AC Lab" top right side of the screen

3. Hover over "My Feature" Note: Here you may update your profile.

4. Select "Committees"

5. Select the link " State Lab/AC view"

6. Select "Files"

7. Select "Add Files"

In the description field enter the name of the document. Please use the new document naming convention located www.aavld.org , 'Accreditation', 'Documents', and select 'Accreditation Program Management'. For naming convention most updated form, (Click here) . Note: Your document may not be larger than 10 MB, if you feel the file is too large to separate the files, please send me the link from Dropbox to [email protected].

8. Select 'Upload' - The webpage will say complete, but it is not complete until you select 'Add File'.

9. Select "Add File" 

 

Naming convention example:

‘2015.CA.LR (2015.07).final’, this is what the first file should look like. Any additional files will be labeled with LRa and so on using the alphabet for any additional files. The 2015 is the year of the original 5-year cycle site visit. The CA is the state you are affiliated with. The LR is Lab Response. Note: Any additional files should be labeled with a, b, c and so alphabet in order.
